I think most do start with 100 end, but you should start with 50 str if you wanted, the way the respec gems work is you can change a bunch over at first before any of the timers start. I also think it's popular to go War/Magic D spec, I was macroing some with a lvl 54 mage that I had and I switched him over to BM spec, but really only War spec'd is important in the beginning since it's your main dmg.
On that AC Utilities page at the bottom there are some stand alone programs, I downloaded Cleaners AC Calculator and it is a decent program and can help you figure out how to spend the skill points and design a template.
